Social Worker

Salary range: Grade 10 £45,123 £47,262 p.a. plus fantastic benefits

Hours of work: 36 hours per week

Location: Croydon

As a Social Worker within the Transitions Service (18-25 years), you will support young adults with disabilities, autism, learning disabilities, mental health needs and complex needs as they transition into adulthood. You will adopt a strengths-based, person-centred approach, working collaboratively with individuals, families, carers and partner agencies to promote independence, wellbeing and positive outcomes.

Key Responsibilities

Undertake proportionate, strengths-based assessments in accordance with the Care Act 2014, identifying individual strengths, aspirations, eligible needs and desired outcomes.

Work collaboratively with young adults, families, carers and professionals to co-produce outcome-focused support plans that maximise independence, choice and control.

Provide information, advice and guidance on community resources, services, support networks and relevant legislation to promote wellbeing and self-determination.

Develop and maintain effective multi-agency and multidisciplinary relationships to ensure coordinated and holistic support.

Identify, assess and manage risks, ensuring appropriate safeguarding measures and risk management plans are implemented.

Complete Mental Capacity Act 2005 assessments, Best Interests decisions and undertake duties under the Care Act 2014, Mental Health Act 1983 (as amended) and other relevant legislation.

Produce high-quality, analytical and evidence-based assessments, care and support plans, reviews and case recordings in line with statutory and organisational requirements.

Promote individuals' rights, strengths, dignity, inclusion and participation within their communities.

Support young people and their families through the transition from Children's to Adult Social Care, ensuring a smooth and person-centred transition process.

About You

You will be a skilled and motivated Social Worker with a sound understanding of strengths-based practice and relevant legislation. You will be committed to empowering young adults to achieve their goals, develop independence and live fulfilling lives within their communities. You will bring strong assessment, risk management and partnership-working skills, alongside the ability to balance statutory responsibilities with a creative and community-focused approach to support planning.
