The Role: Experienced Social Worker
As an Experienced Social Worker, you will play a key role in delivering high-quality, strength-based practice within Adult Social Care.
Key Responsibilities
- Undertake strength-based assessments and care planning
- Provide guidance and informal supervision to Social Workers and Health & Wellbeing Assessors
- Participate in the Duty Senior rota, including allocating work and offering professional advice
- Support colleagues to deliver effective and person-centred interventions
- Work collaboratively across multi-disciplinary teams
LIFE Team (Living Independently for Everyone)
This role sits within our LIFE Team, focusing on hospital discharge through the Discharge to Assess (D2A) pathway.
You will:
- Support individuals to return home safely following hospital discharge
- Deliver short-term, fast-paced interventions that promote independence
- Conduct reablement-focused assessments in people’s own homes
- Work within an integrated, multi-agency service
- Participate in a rota system, including some weekend working
About You
We are looking for someone who:
- Is a qualified Social Worker with post-qualifying experience
- Has experience in adult social care and complex casework
- Is confident in supporting and mentoring colleagues
- Demonstrates strong professional judgement and decision-making
- Is committed to strength-based, person-centred practice
- Thrives in a fast-paced and evolving environment
- Brings creativity, resilience, and a collaborative mindset
