The Learning Disability Team is based at Sessions House in Surbiton, although the Team has adopted RB KIngston's 'Hybrid' working style. Within this, staff work remotely and attend visits and the office when necessary, including for opportunities to work alongside colleagues and to attend monthly face to face team meetings. A Team Charter is in place in order to monitor and evaluate how staff work within this hybrid model. The LD Team consists of a Team Manager, 2 Senior Social Workers and a number of social work and support coordinator roles. As part of the Team, we also have a Transition Team with a Senior Transition Co-ordinator and social workers. The Team is a very stable team and has a wealth of experience, skill and knowledge. Requirements of the assignment / Key deliverables: We are looking to recruit to an interim Transitions Social worker following notice given by one of the permanent worker, this is to support our work. Social Workers – to focus on a number of Care Act assessments of people living in a local setting which has both supported living and residential care placements.
