Assistant Team Manager Role and Senior Physiotherapist Role

To work in a Reablement Team

Assistant Team Manager Role –

* Office visits would only have to be if required- they may have to be on a managers rota where they have to be in the office for 3 days in a week every 3 months and if a joint visit is required they would need to make themselves available

* Supervising Therapists

* Rare Visits to the Office/Mainly working from home

Senior Physiotherapist Role

* Physiotherapist required to assess patients in their own home following hospital discharge and to provide therapy input leading to greater independence and quality of life and decreased dependency on care support. Needs to be able to work with clients on ADL tasks as well as mobility, strength and balance, be able to contribute to MDT discussions about level of care needed and long term care planning

* Working from home with visits required

These posts are for a minimum of 6 months
